## A virus watched more closely than almost any other, and a crossing that has not happened.

### Seventy-one, and no confirmed chain between them

The US human tally is small and specific. CDC records 71 human A(H5) cases in the United States since February 2024 — seven caught by national flu surveillance, sixty-four by monitoring people who worked near infected animals. The agency's own reading of that count is a plain sentence carried on the same page: the current public-health risk is low, while it keeps watching and works with states to monitor people with animal exposures.

No known person-to-person spread has occurred with the A(H5N1) viruses circulating in birds in the United States and globally, CDC says. Read that line the way the source writes it — an absence of confirmation as of the stated date, not a claim that such spread cannot happen. The count is best read as a floor: nearly all confirmed US infections have been in dairy and poultry workers with direct exposure, and farm-worker testing is a known surveillance gap.

### Three ledgers, and the discipline of not merging them

There are really three counts in this file, and the whole story turns on keeping them apart. The first is human cases — 71 in the US, a thousand worldwide since 2003. The second is animal outbreaks, a barn-and-flock ledger the US Department of Agriculture keeps one weekday at a time. The third is the watching: between February 2022 and 30 May 2026, CDC reports, at least 33,100 people were monitored and at least 1,340 tested for novel influenza A after animal exposure.

Those are surveillance-effort metrics, not infections — most monitored people are never infected. The tens of thousands are not a hidden outbreak; they are the reason a single human-to-human link would be seen at all. Merge that number with the 71, and you have invented an epidemic that the source does not report.

### The barn count that only one agency still keeps

The animal side is a different ledger with a different shape. Peer-reviewed work published in a CDC journal in January 2026 logs more than a thousand H5N1-infected dairy herds across the United States, most concentrated in California; the American Veterinary Medical Association's tracker lists nineteen states with H5N1 confirmed in dairy cattle. The 2024 jump into cattle gave the virus a mammalian host on a scale it had never held before.

A June 2026 newspaper reading of USDA's live dashboard put the running total higher — 1,154 affected herds since March 2024, with forty new Idaho detections in a single trailing month — but those figures come from the newspaper, not the agency, and could not be checked: the APHIS dashboard was unreachable throughout the reporting, and the same article contradicts itself on whether nineteen or twenty states are affected. The verified floor is what stands, and it is a barn count, kept in its own column.

### A thousand cases in twenty-three years, and the fortnight that read as routine

Widen the frame to the world and the human ledger lengthens but does not accelerate. WHO's cumulative table counts every laboratory-confirmed human H5N1 case reported since 2003; as of the data date printed on the current version, 8 May 2026, the total is 1,000 cases and 479 deaths from twenty-five reporting countries. The table is overwritten in place — the figure moved from 993 to 997 to 1,000 across three 2026 revisions — so any number must be cited to the printed data line, not the file name.

Zoom back in to a single fortnight and the surveillance reads as routine. In WHO's Western Pacific bulletin for 5 to 18 June 2026, no new human H5N1 case was reported to the region; the last traced to Cambodia with symptom onset on 15 April. The only new human infections were two children with the milder H9N2, both linked to poultry, neither to another person. WHO's bottom line is stated as an absence: no sustained human-to-human transmission identified, and sustained spread currently considered unlikely, even as the zoonotic threat stays elevated because the viruses keep spreading among birds.

### How to read the risk

The honest register here is neither alarm nor all-clear, and it splits along a single line: exposure, not geography. ECDC, assessing the clade circulating in Europe, rates the risk low for the general public and low-to-moderate for those exposed to infected animals; CDC rates it moderate-to-high for people in direct contact with infected animals while calling the general-public risk low. Both halves are official and simultaneously true for their respective populations.

On the food side, FDA files the dairy outbreak as a testing ledger: it is confident pasteurization inactivates H5N1 and that the commercial pasteurized supply is safe, with 464 products sampled and none carrying viable virus. Retail surveys found genetic fragments in roughly a fifth of samples but no live virus in any — fragments the agency is explicit are not infectious virus — while raw, unpasteurized milk it treats as a separate, higher-risk category. The tension the dossier holds is exactly this: a virus watched more closely than almost any other, and a crossing that the sources, on every dated snapshot, record as not yet confirmed.
